Community Primary Care - Alluvial
A primary care physician or PCP, is a health care professional who provides both the first contact for a person with an undiagnosed health concern as well as continuing care for a variety of medical conditions. PCPs are often our first stop for medical care. Primary care physicians provide preventive care, teach patients how to make healthy lifestyle choices, diagnose and treat common medical conditions and make referrals to specialists when needed.
Here at Community Primary Care – Alluvial, we want to build lasting relationships with our patients to help them navigate and manage their overall health – and ultimately stay healthy.
