Insurances Accepted

Your insurance policy is a contract between you and your insurance company. As such, it’s important for you to know the details of it, including co-payments, deductibles and other provisions.

Community Health Partners member practices work with a variety of insurance plans that we bill directly. If you’re covered by an HMO or PPO, your plan may have special requirements such as pre-authorization for certain procedures. Community Health Partners physicians currently accept the following insurances.

Billing Questions

If you received a statement/invoice or need information regarding an amount you owe, please do not hesitate to call Community Health Partners at (559) 230-8500. For hospital-based care (inpatient physician care and/or anesthesia), please call (559) 603-7304. We will work with you and your provider’s office to address any issues you may have.

Sending your Payment

Please do not send written correspondence with your payment. When sending your payment, please enclose your statement and a check or money order with your account number written on it.
