Having surgery can bring a lot of questions. Our Community Perioperative Assessment Clinic helps you feel informed, prepared and confident before your procedure. We provide a comprehensive pre-surgery evaluation designed to identify potential risks early, coordinate your care and help ensure you're ready. By taking a proactive approach, we can help reduce delays, improve recovery and support the best possible outcomes.



What to expect during your visit

Most visits take approximately one to two hours. During your visit, you may:


  • Meet with a registered nurse and a provider
  • Review your medical history, medications and allergies
  • Discuss your upcoming procedure
  • Complete any needed testing or receive orders for additional testing
  • Receive personalized instructions to help you prepare for surgery
  • Discuss advance care planning, if needed


Before you leave, you'll receive any instructions and information needed to help you prepare for surgery day.

What to bring to your visit

Please bring:


  • A photo ID
  • Your insurance card
  • A complete list of medications, including prescriptions, over-the-counter medications, vitamins and supplements
  • Information about any medication allergies
  • Contact information for your primary care provider
  • Any advance directive or power of attorney documents, if available
  • A list of questions you'd like to discuss
